Mission

Identify all Adams 12 students and families who are eligible but not enrolled in Medicaid or Child Health Plan Plus (CHP+) programs while increasing access to needed community resources.

Vision

The Adams 12 Student and Family Outreach Program strives to ensure that all Adams 12 students have health insurance and access to needed community resources. Our goal is for all Adams 12 students to be healthy and ready to learn, enabling them to succeed in all of their academic and social endeavors.
